Prescott crash kills father and kids; third child critical

PRESCOTT, Arizona — A deadly head-on crash on Williamson Valley Road near Levie Lane, northwest of Prescott, took the lives of Jesse Forman of Chino Valley and two of his young children on March 22, 2025, the Yavapai County Sheriff’s Office reported. Deputies got there just after 3 p.m., finding Forman and one child dead at the scene, while another child died on the way to a hospital. Police probe the Prescott crash that left a third child in bad shape.

It happened when Forman, driving a Toyota 4-Runner with his three kids and their mom, crossed double lines and hit a Jeep SUV head-on. The wreck hurt three adults—Forman’s wife and the Jeep’s passengers—who were flown to hospitals in Flagstaff with serious injuries. The last child made it to a Phoenix hospital by ambulance, still in critical condition. Five medical choppers showed up to help, showing how big this mess was.

Police don’t know yet why Forman swerved over the line, and they’re checking if speed or drinking played a part in the Prescott crash. Williamson Valley Road’s been a headache for the Yavapai County Sheriff’s Office, with folks complaining about wild drivers lately. Sheriff David Rhodes promised more patrols, saying safety’s a top job after this awful wreck shook everyone up.

The road’s a quiet one, mostly locals and some weekenders, but this crash hit hard. Rhodes thanked the first responders who jumped in fast, even if it couldn’t save everyone. More news will come as they figure it out.
